Diagnosis

1.History
Have a history of hypertension.

2. Clinical manifestations
In the heart function compensation period, only the general symptoms of hypertension; when the heart function is not fully compensated, the symptoms of left heart failure may occur, the light person only has difficulty breathing after exertion, and the severe person has squat breathing, cardiogenic asthma. Even acute pulmonary edema occurs; patients with chronic disease can develop right heart failure and eventually lead to heart failure.



3. Physical examination
It is found that the apex beat is enhanced, the heart is expanded to the left, and the second heart sound in the aortic valve area can be metallic. The pulmonary auscultation area can have second heart sound hyperactivity due to pulmonary hypertension, apical area or (and) aorta. The valve area can be heard in the systolic murmur of grade II-III/IV, and the apex of the left heart failure can be heard and diastolic. In the case of whole heart failure, the skin mucous membrane is severely cyanotic, jugular vein engorgement, hepatomegaly, edema, and chest and ascites.

4. Laboratory inspection
Electrocardiogram examination showed unilateral or bilateral ventricular hypertrophy and/or strain, P wave broadening or notch, P wave terminal potential (PTF-V1) increased in V1 lead, various arrhythmia and so on. Chest X-ray examination has aortic tortuosity, left ventricle or whole heart enlargement, lung septal line, pulmonary congestion and so on. Echocardiography showed unilateral ventricular or bilateral ventricular hypertrophy, left ventricular diastolic dysfunction, and reduced ejection fraction.
